Privacy Policy
DNG Supports Pty Ltd (ABN 80 691 265 988) provides NDIS support coordination. To do that well, we need to collect some personal information. This policy explains what we collect, why, and how we look after it. We follow the Privacy Act 1988 (Cth), the Australian Privacy Principles and the NDIS Act 2013.
What we collect
- Contact details: your name, phone number, email and address.
- NDIS details: your NDIS number, plan dates, funded supports and goals.
- Health and disability information relevant to coordinating your supports. This is sensitive information and we treat it with extra care.
- Details of the people around you: nominees, guardians, family members and other providers, where relevant to your supports.
How we collect it
Most information comes from you or your representative, through our referral form, by email, or in conversations with your coordinator. Our referral form is run by Astalty, an Australian NDIS software provider, and information you enter there is stored in our Astalty system. Sometimes we receive information from others involved in your supports, like your LAC, plan manager or another provider, and only so we can do our job.
Why we collect it
- To provide support coordination under your NDIS plan.
- To connect you with providers you choose.
- To claim for our services under your plan.
- To meet our obligations to the NDIA and the NDIS Commission.
- To respond when you contact us.
We do not sell your information or use it for marketing.
Who we share it with
We share your information only with your consent, or where the law requires or allows it: with providers you choose to work with, your plan manager, the NDIA, the NDIS Commission, and emergency services if someone is at serious risk. We ask for your consent when you join us and you can change or withdraw it at any time.
How we protect it
Your records are kept in secure, password-protected Australian systems with access limited to the people who work with you. We keep records only as long as the law requires, then dispose of them securely.
Our website
Our website does not use advertising or tracking cookies. Our hosting provider keeps standard server logs to keep the site running. If you use the contact form, your message is delivered to our email inbox by FormSubmit, a form delivery service, and is not stored anywhere else.
Access and corrections
You can ask to see the information we hold about you, and ask us to correct anything that is wrong, by emailing info@dngsupports.com.au. We will respond within 30 days.
Concerns
If you think we have mishandled your information, tell us first and we will try to put it right. If you are not satisfied, you can contact the Office of the Australian Information Commissioner at oaic.gov.au or 1300 363 992.
